Visio Add-in for WBS Modeler - Ribbon does not Appear

I downloaded the 32-bit version of the WBS Modeler add-in for Visio and the ribbon for it does not appear.

I have:

1. Checked my version - it's Visio Premium 2010 32-bit
2. Tried enabling the add-in - does not appear, list is blank
3. Tried installing the Office PIA redistributable 
4. Tried installing .NET Programmability Support
5. Tried repairing the program and then worked through lines 2-5 above
6. Tried uninstalling and reinstalling the program and then worked through lines 2-5 above
7. Tried restarting my computer

Is there anything else I can try?  I really want to be able to try this feature and it's getting frustrating.
